A major health organization in Wales is seeking a Band 7 Clinical Pharmacist to join their dynamic pharmacy team. This is a unique opportunity to make a real difference for patients with complex needs and multiple comorbidities, with a focus on polypharmacy, frailty reviews, and holistic care. You will work closely with multidisciplinary teams, attending ward rounds and community resource team meetings, and contribute to optimising patient outcomes across the care pathway.

This is a full-time (37.5 hours) position based within Central BCU, covering both Conwy and Denbighshire.
