Job description
Position Summary

A permanent Band 6 Clinical Nurse Specialist – Palliative Care (22.5 hour per week, 0.6 WTE) based at Nursery Park in Ashington, Northumberland. The role is part of the Specialist Palliative Care Community Team, delivering specialist palliative care to patients with complex needs in community settings, including 7 days of extended working hours (9am–7pm) and weekends/bank holidays.

Responsibilities
  • Provide specialist palliative care for patients with life limiting illnesses in community settings.
  • Develop partnerships with members of Multidisciplinary Team in the Trust in order to provide a seamless service of palliative care, to enable the patient to be maintained in an environment of their choice.
  • Provide clinical professional support, resources and education when necessary to members of the Multidisciplinary Team.
  • Work as an autonomous practitioner within a multidisciplinary environment, ensuring adherence to professional codes of conduct.
  • Work with other professionals to promote and develop the ethos of Palliative Care within the Trust and the local community.
Qualifications & Experience
  • Band 6 Clinical Nurse Specialist or equivalent clinical experience in palliative care.
  • Strong leadership and communication skills with the ability to work as an integral member of a multidisciplinary team.
  • Experience managing patients with complex, life‑limiting illnesses and working in partnership to enable care in the patient’s preferred setting.
  • Substantial palliative care experience is an advantage.
