You will bring your expertise inspecialist palliative care to patients across the city, offering highqualityclinical care, guidance, and support

This is a unique opportunity for an autonomous practitionerwho thrives in community settings and enjoys managing a varied and rewardingcaseload. Youll work closely with our multidisciplinary team to deliverexcellent patientcentred care and contribute to the ongoing development of ourservices.

What Youll Do

  • Work independently while remaining an active partner within a skilled multidisciplinary team.
  • Support learning and development through informal and formal teaching.

What Were Looking For

  • Level 1 Adult Registered Nurse & prescriber or currently completing the NMP course; NMC registered.
  • Postregistration qualification in palliative care, or willingness to undertake one.
  • Significant experience as a senior nurse, ideally with teaching responsibilities.
  • Advanced experience in an acute hospital or hospice at Band 7 (or equivalent).
  • Excellent communication and interpersonal skills.
  • Own car and full UK driving licence.

Job purpose

To provide specialist palliative care nursingservice to patients, carers, and health care professionals. The role supports and enables safe andeffective strategic community palliative and end of life care servicedevelopment. The role will involve working in a community setting.

Expert Clinical Practice

Giveencouragement, support, empathy, time, reassurance and advice to patients,families and carers to enable them to explore and express feelings and issuesof concern promoting informed choice.

Demonstrateadvanced practice in holistic assessment, development and evaluation ofindividual specialist nursing care programmes involving patients, families,and carers as appropriate.

Facilitatethe management and support of palliative care patients to ensure the pathwayis seamless and of a high quality

Topromote autonomy and advocacy to ensure the delivery of appropriate care

Workingas an autonomous CNS, demonstrating a high level of empathic andinterpersonal communication skills for patients and carers needing palliativecare

Demonstrateadvanced knowledge as an expert practitioner in palliative care by use ofinnovative practice and skills
