Clinical Nurse Specialist Community Palliative Care

An exciting opportunity has arisen for an experienced Palliative Care CNS to work within the community setting covering a 7 day service 08:00 - 20:00 .

We are looking for an experienced, autonomous, and innovative nurse to join the integrated specialist palliative care nursing team.

This is a Band 7 community-based palliative care CNS post, working alongside, Community Nursing Services, Consultants, Therapists, and wider MDT to ensure that the service and team are providing expert high quality patient focused care to advice and support patients, families and Health Care Professionals.

You will be working closely with GP's, Community Teams and Acute Trust staff. As an integral part of community nursing, you will support community teams to feel confident and competent to deliver good palliative and end of life patient centred care and providing expertise for patients with palliative care needs across the borough.

You will be expected to have a commitment to and progressive work within the field of palliative care. Additionally, you must be able to demonstrate excellence in clinical practice, acting as a resource for information and education, while working alongside the other specialist services.

Experience
  • Relevant post registration experience at band 6 or above in this or appropriate speciality
  • Previous experience in a post involving clinical and managerial responsibilities.
  • Previous experience of teaching and developing teaching materials
  • Demonstrates evidence of well-developed clinical practice
  • Ability to develop new ways of working
  • Ability to deal with complex patient care including symptom control and psychological support
  • Able to manage own caseload
  • Conversant with the key principles of clinical governance
