Job summary

Marie Curie seeks a confident Clinical Nurse Specialist for a new Primary Care Network project in Exeter. The role focuses on identifying and supporting people approaching the end of life, working across primary care, community and acute settings to connect patients with appropriate care and reduce hospital admissions.

The post requires current NMC registration as an Adult Nurse and experience in complex health needs or palliative care, with strong assessment and multidisciplinary collaboration

Qualifications

  • NMC registration as an Adult Nurse is required.
  • Experience supporting people with complex health needs and/or end-of-life care.
  • Strong clinical assessment and decision-making skills.

Role Overview

We're excited to offer an opportunity for an experienced Clinical Nurse Specialist to join an innovative new Primary Care Network (PCN) project in Exeter. This role will play a key part in improving how people approaching the end of life are identified, supported and connected to the care they need. Working across primary care, community and acute settings, you'll help shape a developing service that aims to improve patient outcomes, reduce unnecessary hospital admissions and support people to receive care in the place that is right for them., As a Clinical Nurse Specialist, you will work closely with Primary Care Network colleagues, community services and wider multidisciplinary teams to improve the experience of people approaching the end of life. You will:



Key Responsibilities


  • Support the early identification of people who may benefit from End-of-Life Care support.

  • Undertake holistic clinical assessments and work collaboratively to coordinate care.

  • Facilitate advance care planning discussions with patients, families and carers.

  • Help improve communication and continuity of care across services.

  • Support admission avoidance and contribute to effective discharge planning.

  • Contribute to service development, quality improvement and best practice initiatives.

  • Promote person-centred care that reflects individual preferences and priorities.

  • This is a fantastic opportunity to influence how End of Life Care is delivered across Exeter while working collaboratively with healthcare professionals across multiple services.

  • This role is to support proactive identification of patients, community-based care and emergency department discharge support across Exeter.

  • Help shape and develop a new model of End-of-Life Care.

We're looking for a confident and compassionate registered nurse who enjoys working collaboratively and is passionate about improving patient outcomes.



You will have:


  • Current NMC registration as an Adult Nurse.

  • Experience supporting people with complex health needs and/or End of Life Care needs.

  • Excellent clinical assessment and decision-making skills.

  • Experience of care planning and coordinating care across services.

  • Strong communication and relationship-building skills.

  • Experience working within multidisciplinary teams.

  • A commitment to delivering high-quality, person-centred care.

  • An interest in service improvement and improving patient experiences.

  • Experience working in Specialist Palliative Care

  • Proven experience in providing care to patients with complex needs, ensuring comfort and quality of life during terminal illness.

  • Experience in advanced care planning and respect discussions, Ability to connect with patients and their families on an emotional level, providing support and understanding during difficult times.

  • Registered Nurse on NMC register (Adult) Must be a registered nurse with the Nursing and Midwifery Council, ensuring adherence to professional standards and regulations.
