The purpose of this role oversee referrals into the Service Centre, helping young people and families receive the right support at the right time.
The main areas of the job will include

- Ensure all referrals are promptly screened for risk and safeguarding concerns.
- Allocate cases to colleagues for information gathering
- Support clinical decision making
- Provide clinical guidance, sign posting and advise.
- Offer telephone consultation and advice to families and professionals
- Work closely with other colleagues from Northpoint and partner agencies.
Referral Management
- Ensure all referrals are risk screened within a working day of receipt.
- Co-ordinate and allocate referrals for information gathering.
- Ensure appropriate and timely plans are identified and implemented, including where clinical risk and safeguarding concerns are present.
- Ensure clinical pathways and escalation processes are followed in line with agreed guidance.
- Help to ensure that communication of referral decisions is clear, accessible, and helpful.
Information Gathering & Record Keeping
- Help to ensure that information collected is relevant, focused, and in line with service guidelines to enable informed decision-making.
- Coordinate record keeping of information gathered, next steps, and decisions made.
- Monitor and record patient-related information onto electronic patient management system(s) in a timely and accurate fashion in line with information governance protocols.
- Maximize efficient use of systems and ensure data integrity.
Communication & Consultation
- Act as a first point of contact for queries from professionals and families, responding in a clinically appropriate and professional manner, escalating to senior members of the team where required.
- Offer and co-ordinate consultation and respond effectively to families and professionals who may want to explore support options for a young person without the need for a referral.
- Coordinate meetings as relevant, ensuring action-led outcomes.
Knowledge & Systems Development
- Maintain a good knowledge of available services in the local area, including online resources, apps, and websites.
- Apply systems thinking to help explore and understand interconnected needs and opportunities for user-centred support to children, young people, and families.
Northpoint is a BACP-accredited service operating Service Centres for single point of access (SPA) and central point of contact (CPOC) services in West Yorkshire. Our Service Centres facilitate referrals and requests for support related to mental health and emotional well-being and ASD/ADHD requests for assessment. Our model is digitally enabled to provide an efficient and informed service user experience, connecting people to the right local resources and support, at the right time, in line with their individual goals.
In addition to our Service Centres, Northpoint provides school-based support and therapeutic interventions and operates as the lead provider in select local mental health partnerships in collaboration with Integrated Care Boards (ICBs) and NHS Trusts.
