The Forensic Community Transitions Pathway is a newly developed pilot service to support people in the Shropshire, Telford and Wrekin area that have left secure forensic services and are transitioning from MPFTs Forensic Intensive Recovery Support Team (FIRST) into a community mental health service. The team will provide specialist assessment, formulation and treatment to support them during this transition to gain/regain their skills and confidence to live successfully in the community.
The service is currently looking for a Registered Mental Health Nurse / Social worker / AHP to join the service on a temporary contract until 31/03/2028, full-time post (37.5 hours per week).
This role and service will need to develop strong working relationships with other services both internally and externally and is co-located with other mental health teams as the goals of the service are to support individuals with rehabilitation and re-integration into the community.
This is an excellent opportunity to develop leadership and service development skills and also to understand the wider health and social care system in Shropshire, Telford and Wrekin.
This post is being advertised as a temporary contract until 31/03/2028. However, secondment opportunities would be considered for internal MPFT employees upon prior agreement of your current line manager.
