Responsibilities

• Lead PHBs implementation with specialist guidance.
• Provide training on CHC, PHBs and related areas.
• Ensure timely reviews for Continuing Care and Funded Nursing Care.
• Support investigations in the appeals process and respond to queries.
• Promote collaboration across healthcare, acute services and social care.
• Identify and manage safeguarding concerns including Best Interest Assessments.
