Job summary

Pharmaxo in the United Kingdom is seeking a dedicated home care nurse to support patients in clinics, residential settings, and patients' homes. You will perform observations, administer treatments, and ensure safety and compassionate care.

You will monitor vital signs, educate families, maintain confidential records, and work with multidisciplinary teams to prioritise patient safety. A valid UK driving licence is essential and substantial post-registration clinical experience is expected.

• Experience of administering S/C, IM or IV medications
• SACT experience

Responsibilities

• Perform patient observations and ensure that any patient is fit to proceed with their treatment
• Administer medications and treatments as prescribed by healthcare providers
• Monitor patient vital signs and report any changes to the medical team
• Provide compassionate support to the patient and family
• Educate patients and their families to promote understanding and independence.
• Maintain accurate clinical records and ensure confidentiality of patient information
• Collaborate with interdisciplinary teams ensuring that patient safety is paramount.

As more and more patients are treated outside a traditional hospital setting, the role of the home care Nurse is developing. We work in a variety of environments such as clinics to residential accommodation and patients' own homes. We are able to treat individuals at school or even in a work environment. Our nurses help the NHS meet the needs of elderly, disabled or vulnerable patients who may not be able to easily visit the hospital. We are able to help support our patients to become more independent in the management of their treatment and to make the whole treatment journey more convenient. You will need to be kind, compassionate and caring with a desire to provide excellent patient experience and safe and effective care. Duties
