A leading NHS healthcare provider in Liverpool is seeking a motivated Thrombosis/Anticoagulation Nurse Specialist to join its Clinical Haematology Team. The successful candidate will provide high-quality care to patients, lead clinical audits, and mentor junior staff. This role involves working in both hospital and community settings to support patients needing anticoagulation therapy, ensuring best practices and evidence-based care are maintained.
An exciting opportunity has arisen for a motivated and dynamic Nurse to join the Clinical Haematology Team. As a Thrombosis/Anticoagulation Nurse Specialist, you will play a key role in providing high-quality, compassionate care to patients. You will work alongside multi-disciplinary teams to deliver care that promotes the health and wellbeing of patients. The successful candidate will work across the NHS University Hospitals of Liverpool Group in hospital and community settings. They will assess patients, plan, deliver and monitor the care and management of patients who require anticoagulation therapy.
Provide expert clinical nursing care to patients with haematological conditions. Assess patient needs, develop, and implement care plans in collaboration with the multidisciplinary team. Offer patient education and support regarding treatment options, self-management strategies, and lifestyle changes. Monitor patient progress and adjust care plans as necessary, ensuring best practices and evidence-based care are followed.
