Responsibilities

  • Process and code incoming clinical correspondence using SNOMED CT codes.
  • Scan and attach documents to patient records in SystmOne.
  • Organise and manage workflow documents efficiently.
  • Input accurate clinical codes to maintain patient records.
  • Update patient problem lists and Summary Care Records where appropriate.
  • Forward correspondence to the appropriate clinician when required.
  • Ensure all coding is completed accurately and within agreed timescales.
  • Input and extract data to support practice reports, audits, patient safety and QOF income.
  • Highlight timely actions and changes to patient care to the relevant clinician.
  • File, store and photocopy documentation as required.
  • Action all incoming correspondence, emails and post in a timely manner.
  • Answer incoming telephone calls, transfer calls or deal with requests appropriately.
  • Assist with total triage administration.
  • Conduct system searches as requested.
  • Maintain a clean, tidy and effective working area.
  • Support clinical staff with general administrative tasks as requested.
  • Undertake a broad spectrum of administrative duties commensurate with the role.
  • Act as a chaperone as required.
  • Undertake additional administrative duties to support the smooth running of the practice.

Main Duties of the Job
  • Process and code incoming clinical correspondence, both electronic and paper-based, using standard SNOMED CT codes.
  • Scan and attach documents to patient records using SystmOne.
  • Organise and manage workflow documents efficiently.
  • Input accurate clinical codes to maintain patient records.
  • Update patient problem lists and Summary Care Records where appropriate.
  • Forward correspondence to the appropriate clinician when required.
  • Ensure all coding is completed accurately and within agreed timescales.
  • Input and extract data as required to support practice reports and audits, and to support both patient safety and QOF income.
  • Highlight all timely actions and changes to patient care and management to the relevant clinician.
  • File, store and photocopy documentation as required.
  • Action all incoming correspondence, emails and post in a timely manner.
  • Answer incoming telephone calls, transferring calls or dealing with the caller's request appropriately.
  • Assist with total triage administration.
  • Conduct system searches as requested.
  • Maintain a clean, tidy and effective working area at all times.
  • Support all clinical staff with general administrative tasks as requested.
  • Undertake a broad spectrum of administrative duties commensurate with the role.
  • Act as a chaperone as required.
  • Undertake additional administrative duties to support the smooth running of the practice.
Wider Responsibilities

In addition to the primary responsibilities, the Clinical Coder has the following wider responsibilities:

  • Participate in local initiatives to enhance service delivery and patient care.
  • Support and participate in shared learning.
  • Complete opening and closing procedures in accordance with the duty rota.
  • Support reception staff, providing cover during staff absences.
  • Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on safeguarding adults and safeguarding children.
  • Undertake all mandatory training and induction programmes.
  • Contribute to and embrace the spectrum of clinical governance.
  • Attend a formal appraisal with their manager at least every twelve months.
