Isabellas homes ltd is seeking an experienced Care Coordinator to lead clinical oversight across its supported living services. The role is ideal for a registered nurse with a background in community-based or complex adult care, offering opportunities to drive high-quality, person-centred care in a fast-paced environment.

You will provide clinical leadership, ensure adherence to CQC standards, and collaborate with operational and quality teams to maintain service excellence.

Qualifikationen

• Minimum 3 years post-registration experience in a clinical or care coordination role.
• Experience in supported living residential care or community adult health.
• Strong understanding of CQC, DoLS, MCA, and risk management frameworks.
• Excellent communication, leadership, and organisational skills.
• Ability to travel between services (mileage reimbursed).

Aufgaben

• Provide clinical leadership across supported living services.
• Review and implement care plans for individuals with complex or high-risk needs.
• Act as lead clinical liaison for referrals, transitions and MDT meetings.
• Support service managers to embed best practice and maintain CQC compliance.
• Monitor incident reports, safeguarding alerts and health outcomes.
• Lead on health audits, clinical risk assessments and medication review.
• Train and mentor support staff in trauma-informed high-quality care.
• Work collaboratively with local authorities, NHS commissioners and families.
