Job description
Overview

Are you calm under pressure and passionate about making a difference in your community? We are seeking a Triage Clinical Care Co-ordinator to join our Care Connexions team, providing timely support to people experiencing health or social care crises in their own homes.

Responsibilities
  • Support the triage processes and allocation and co-ordination of the clinical response for Care Connexions.
  • Provide clinical oversight and support to the triage practitioners, helping them triage appropriately and safely and ensuring seamless co-ordination of services.
  • Support timely access to hospital avoidance services including 2 hour Urgent Community Response, Virtual Wards, Home First Service and 3rd sector services.
  • Have experience working across Health and Social care settings with an understanding of hospital avoidance services to ensure the right services are deployed to best support the needs of the patient.
Working Hours

37.5 hours over a 7-day working week, covering 8am-8pm.

Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care. The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
