Job Summary

The Chronic RN is responsible for delivering and coordinating clinically competent hemodialysis care for an assigned group of patients with End Stage Kidney Disease (ESKD). This role involves patient assessment, care planning, intervention, and evaluation to ensure safe and effective treatment. As part of the ESKD healthcare team, the nurse participates in decision-making, education, leadership, and quality improvement initiatives that enhance patient outcomes and support overall clinic operations.

Key Responsibilities
  • Provide safe, effective hemodialysis therapy in accordance with clinical and regulatory standards.
  • Assess, plan, and document patient care activities pre-, during-, and post-treatment.
  • Administer prescribed medications per policy and document all interventions accurately.
  • Educate patients and families on treatment plans, transplant options, and health management.
  • Collaborate with physicians, supervisors, and the interdisciplinary team to review and update patient care plans.
  • Serve as a mentor and resource to Patient Care Technicians, LPNs/LVNs, and other RNs.
  • Respond promptly to medical emergencies and initiate appropriate interventions.
  • Perform laboratory collection, handling, and processing procedures in compliance with protocol.
  • Promote a safe, patient-centered environment that supports quality improvement goals.
Physical Demands and Work Environment
  • Provides direct patient care requiring frequent standing, lifting, and assisting with ambulation.
  • May lift or move up to 30 lbs. (chemicals or solutions) and occasionally up to 200 lbs. with equipment assistance.
  • Exposure to infectious or contagious materials in a healthcare environment.
  • Regular interaction with patients, medical staff, and interdisciplinary team members.
  • Travel may be required for training or coverage at other facilities.
Required Experience
  • Minimum 1 year of nephrology nursing experience within the past two years as a Registered Nurse.
  • Chronic hemodialysis experience required.
  • At least 9 months of RN experience and 3 months in chronic or acute dialysis prior to leadership assignment.
Education and Licensure
  • Graduate of an accredited School of Nursing.
  • Active state RN license in good standing (Michigan).
  • CPR/BLS certification required.
  • Must meet Michigan nursing practice standards for dialysis care.
