Job description

Auckland Medical Group are looking to recruit an experienced and motivated Chronic Disease Nurse.

The post holder will provide and maintain a high standard of chronic disease management for patients with chronic diseases, which includes the diagnosis, monitoring and developement of individual management plans and agreeing these as appropriate with the patient and other health professionals.

The post is for 24 hours per week.

Main duties of the job

To join our nursing team and be responsible for chronic disease management of our patients.

The successful applicant must have excellent communication skills and flexibility is imperative due to the nature of the role

About us

A busy teaching/dispensing practicewith over 15950 patients based over 3 sites. We have 6 partners, 2 salaried GPS, 4 nursepractitioners, 9 practice nurses, 2 nursing associates, 3 HCA, 1 phlebotomist, an in house pharmacy team and a good team of admin andreceptionists.

Job responsibilities
  • Identifying significantabnormalities using a holistic approach to history taking and observationand assessment of physical systems
  • Diagnosis, monitoring anddevelopment of individual management plans; agreeing these as appropriatewith the patient and other health professionals
  • Understand the aims of managementand the pharmacological treatments for chronic diseases
  • Initiate, perform and interpretdiagnostic procedures as appropriate for chronic disease management
  • Support recall procedures,appropriate literature and health promotion, and audit

Management of Medicines

  • Diagnoseand manage both acute and chronic conditions, integrating both drug-andnondrug-based treatment methods into a management plan.
  • Prescribe and review medication fortherapeutic effectiveness, appropriate to patient needs and in accordancewith evidence-based practice and national and practice protocols, andwithin scope of practice.
  • Provideinformation and advice on prescribed or over-the-counter medication, medicationregimes, side-effects and interactions
  • Work with patients in order to support compliance with, and adherence to,prescribed treatments
