Role Overview

Please note - this role is home/hybrid working - ICB wide. The office base will be the closest to the candidates home.

Responsibilities

Our CYPCC Nurses undertake evidence based, holistic needs assessments following the National Framework for Children and Young People's Continuing Care. The assessment process is a collaborative approach with the child and family at the centre, and extensive input from multi‑agency professionals. Utilizing specialist nursing knowledge you will signpost to commissioned NHS services, identify gaps in health care provision, develop and commission comprehensive needs‑led packages of care in partnership with the child/young person, their family, and professionals across health, social care and education. The post holders will provide specialist health advice and contribute to complex case management alongside colleagues in health, social care, and education. You will take an active role in the Education, Health and Care planning (EHPC) process; Care, Education and Treatments Reviews (CETRs) and multi‑agency meetings with local authority Children with Disabilities Services. You will work under the guidance of our clinical Team Leaders and CYPCC Strategic, Operational, and Quality Lead, with access to regular clinical and safeguarding supervision, and opportunities for training and development. We support flexible working, with staff wellbeing and development of paramount importance. We encourage collaboration between our place teams with regular team meetings and informal weekly wellbeing catch‑ups.

Qualifications

The Children and Young Peoples Continuing Care (CYPCC) Team are looking for experienced registered nurses (RN Child, RNLD/RNMH, RN with PHN qualification) who are passionate about improving care for children and young people with complex needs.

  • Experience working in the field of children’s complex health/palliative care/learning disability/mental health is essential.
  • We are particularly keen to attract nurses with experience of working with children/young people/adults with autism and/or intellectual disability and behaviours that challenge.
Working Hours and Location

We have various hours available; our teams include South Bank (North & North East Lincolnshire), North Bank (Hull & East Riding) and North Yorkshire & York. Our CYPCC Nurses are predominantly remotely based and aligned to one of the above teams, however there is an expectation that post holders are willing to be flexible, able to travel for face‑to‑face assessments and support across the teams in line with service need.
