Job summary

Great Western Hospitals NHS Foundation Trust is recruiting a Children’s Community Health Care Support Worker to provide direct care to children and young people with complex medical needs in home and community settings.

You will follow individualized care plans, gain competency through a comprehensive training programme, and work with a supportive nursing team to enable families to rest and ensure safe care in home and educational settings.
