Job summary

HCRG Care Group is recruiting a Children and Young People Practitioner for the Essex Child and Family Wellbeing Service in Basildon. You will work with a multidisciplinary Healthy Family Team to deliver the Healthy Child Programme 5–19 across schools, homes and Family Hubs, supporting at-risk children and those with complex needs.

You will collaborate with School Nurses, deliver targeted health promotion and engage with families to identify and respond to needs, while managing your workload

Qualifications

• Strong interpersonal and communication skills essential for engaging with children, families and schools.
• Excellent time management and organisational abilities to balance caseloads and interventions.
• Proficient IT skills, including Outlook, Word and Excel for case notes and reporting.
• Hold a valid UK driving licence and have access to a car for work.
• Commitment to safeguarding and a child-centred approach in community settings.
• Experience or interest in public health and community-based care is desirable.

Responsibilities

• Collaborate with School Nurses to meet health needs of children and young people.
• Deliver components of the Healthy Child Programme 5–19 in schools, homes and Family Hubs.
• Provide brief intervention and support on emotional wellbeing, relationships, and healthy lifestyles.
• Lead health promotion campaigns through school clinics, assemblies and outreach.
• Work with parents, carers and partners to identify needs and respond effectively.
• Manage workload under supervision and contribute to KPIs and outcomes.

Are you a practitioner looking to grow your experience working with children, young people and families in the community? As a Children and Young People Practitioner, you'll help young people build knowledge of self-care, autonomy and decision making, supporting their health and wellbeing and empowering them to make positive life choices.

Working under the delegation of a School Nurse, you'll deliver components of the Healthy Child Programme 5–19 (up to 25 for SEND) across mainstream schools, homes and our Family Hubs, providing targeted interventions and groups for children and young people at risk of poor outcomes or with complex needs.

HCRG Care Group works in integrated partnership with Barnardo's as the Essex Child and Family Wellbeing Service, supporting families from pre-birth to 19. You'll join a skill-mixed Healthy Family Team of Health Visitors, School Nurses, Support Practitioners and Engagement Workers, in a service rated "Good" by the CQC and focused on outcomes, not just numbers.

Main Responsibilities

As a Children and Young People Practitioner, you'll:

• Work in collaboration with School Nurses to meet the health needs of children and young people, helping them develop self-care, autonomy and decision making, including how to access health services.
• Deliver aspects of the Healthy Child Programme 5–19 pathway as delegated by the School Nurse.
• Provide brief intervention and solution-focused support on issues including emotional wellbeing, exploitation, sexual health, relationships, substance and alcohol misuse, risk taking and healthy lifestyles, in schools, homes or Family Hubs.
• Deliver targeted health promotion campaigns through school drop-in clinics, assemblies and community outreach.
• Work with parents, carers, and statutory and voluntary partners to identify and respond to young people's and families' needs.
• Manage your own workload under the supervision of the School Nurse and contribute to Key Performance Indicators and outcome measures.

Ideal Candidate

We're looking for someone who:

• Has a passion for helping children have the best possible start in life and through key transitional stages.
• Has a keen interest in children, young people and the public health agenda.
• Has excellent interpersonal and communication skills, and is confident speaking to large groups such as school assemblies.
• Has excellent time management and organisational skills.
• Is confident using IT systems, including Outlook, Word and Excel.
• Holds a valid UK driving licence and has access to a car for work.
