Overview

Cheshire East Council is seeking experienced Qualified Social Workers (QSWs) to join our Adult Social Care teams.

This is an excellent opportunity for dedicated professionals to make a real impact by supporting adults to live independently, promoting well-being, and ensuring that care and support needs are met in line with the Care Act 2014.

You will work collaboratively with adults, their families, and multi-disciplinary partners to carry out strengths-based assessments, develop care plans, and review support to achieve positive outcomes.

Responsibilities

• Undertake strengths-based assessments and develop person-centred care and support plans.
• Work in partnership with health professionals, care providers, and voluntary organisations to deliver effective, coordinated services.
• Manage a varied caseload of adults with complex needs, including older people, individuals with physical disabilities, and those with mental health issues.
• Apply the principles of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults legislation.
• Support hospital discharges and community-based interventions where required.
• Maintain accurate and timely case records and ensure compliance with statutory requirements.

Requirements

• Qualified Social Worker (Degree in Social Work, CQSW, DipSW or equivalent).
• Current registration with Social Work England.
• Experience within adult social care, community, or hospital-based social work.
• Strong assessment, decision-making, and communication skills.
• Ability to work effectively as part of a multi-disciplinary team.
• Enhanced DBS check required.
