NHS Norfolk and Suffolk Integrated Care Board is recruiting Band 6 Registered Nurses (Adult, Mental Health or Learning Disability) for NHS Continuing Healthcare. Based in Norwich, you will assess, case manage and support decision-making for complex needs, within a CHC team spanning Norfolk and Suffolk.

The role offers full-time, part-time and flexible working, with a base in Norwich or Ipswich. You will use clinical judgement and leadership to deliver person-centred care and robust eligibility

As a Nurse Coordinator, you will use your clinical expertise to undertake complex assessments, case manage individuals eligible for NHS Continuing Healthcare and support the delivery of the Discharge to Assess model. You will work alongside individuals, families and multidisciplinary professionals to ensure fair, robust and person-centred decision making for people with significant and complex health needs. • Co-ordinate and undertake timely, comprehensive and person-centred assessments, using the appropriate national and local tools for NHS CHC and NHS-funded Nursing Care.
• Gather, evaluate and triangulate clinical, functional and contextual evidence from the individual, their representative and relevant professionals, identifying gaps or inconsistencies and taking proportionate action to resolve them.
• Prepare clear, accurate and evidence-based assessment reports and eligibility recommendations, demonstrating the application of professional judgement and the relationship between assessed needs, evidence and the relevant national criteria.
• Participate in multidisciplinary team meetings, case conferences and decision-making processes, ensuring the individual and their representative are enabled to understand and contribute to the process.
• Complete and review person-centred, outcome-focused care and support plans for people eligible for NHS funding, ensuring that commissioned care is sufficient to meet assessed needs and that identified risks are appropriately managed.
• Support individuals to understand and access Personal Health Budgets where applicable, including contributing to safe and lawful support planning and review arrangements.
