Job summary

Qualifications

Responsibilities

• Act as CHC commissioning nurse for patients funded by NHS GM and independent sector
• Identify and case manage clients eligible for NHS funding; chair eligibility assessments and reviews
• Provide expert advice to professionals on NHS Continuing Healthcare and NHS Funded care packages
• Complete restitution process for all claims; present findings to NHS GM panels
• Contribute to quality monitoring of care home providers
• Support the duty function of the service
