Join Our Team as a Support Facilitator (Home Care) - Make a Real Difference Every Day! Are you passionate about helping people live their best lives? Staffordshire County Council's Provider Services team is looking for dedicated Support Facilitators to deliver person‑centred reablement care in the community. You'll be part of an innovative and empowering service that supports individuals to thrive in their own homes.

Our service runs 7:00 AM to 10:00 PM, and we need team members who can start early and work flexibly to meet the needs of those we support. Applicants must live within 30 minutes of our base locations ST5 1BL, ST13 6DW, ST16 2DH & DE14 1AHto ensure timely response and continuity of care.

This role requires travelling within the community, North & East Staffordshire, and so a full UK driving licence and access to your own vehicle is essential.

We have a casual position available in each area. There is a business need that all early shifts will start at 7 am. We require our team to work flexibly, which means you may be required to work other days to complete training.

  • We provide full paid training and ongoing development to enable you to pursue your career aspirations.
  • For travel, we will reimburse you at £.45/mile, paid from either Newcastle, Leek, Stafford or Burton Library.
  • Supervision and support to enable you to provide excellent care.

This post is subject to an enhanced DBS check and will require a pre‑employment health assessment as part of the onboarding process.

Main Responsibilities

Working flexibly across various locations in North Staffordshire such as Newcastle under Lyme, Moorlands, Stafford & East Staffs, you will provide an excellent level of care to enable the people we support to have a comfortable and dignified life within their own homes.

As a Support Worker, you will:

  • Provide person‑centred reablement care in people's homes, respecting dignity and promoting independence.
  • Support individuals with daily living tasks, personal care, and medication administration.
  • Communicate effectively with individuals, families, and colleagues to build positive relationships.
  • Respond flexibly to changing needs and embrace digital tools to improve service delivery.
  • Maintain accurate records and comply with health, safety, and safeguarding standards.
  • Undertake manual handling tasks as required.
  • Drive (using your own vehicle) to deliver care across the local area.
  • To complete interventions using Assistive Technology.

Interviews will be held: Week commencing 15th December 2025.
