Shropshire Community Health NHS Trust is seeking a Caseload Holder to join the South East IDT team delivering urgent and planned care in the community on a permanent basis. The role involves managing a caseload with a skilled team to provide timely, evidence-based care outside hospital settings.

The post requires RN registration with the NMC and current community nursing experience. We value proactive, flexible practice and ongoing CPD to maintain high standards of patient care.

Job summary

An exciting opportunity has arisen for a Caseload Holder to join the South East IDT team in the provision of Urgent and Planned Care to patients living in the community, on a permenant contract. We are a proactive and forward thinking team, who constantly strive for the delivery of excellent patient care and modern ways of working. The multidisciplinary teams provide nursing and therapy services offering an alternative to hospital admission, supporting timely discharge. We enable our patients to realise their potential independence in their most appropriate place of care.

Main duties of the job

As a Caseload Holder you will manage a designated group of patients to ensure that they receive timely and evidence based care. You will have a skill mixed team to support the delivery of this care. You will need to be qualified to RN level, be registered with the NMC and be able to demonstrate current clinical experience in the Community setting. We are committed to clinical support, CPD & training.

Person Specification

Training and Qualifications

• Level 1 Adult Nurse Registration with Nursing and Midwifery Council (NMC)
• Diploma/Degree Specialist Practice District Nursing or alternative post registration qualifications in a relevant discipline or portfolio of significant relevant experience
• Community Practitioner Nurse Prescribing (V100 or V150) or V300 non-medical prescribing or willing to work towards this

Experience

• Wide range of nursing experience within a community setting
• Evidence of personal and professional development

Knowledge and Skills

• Advanced communication, negotiation, decision making and organisational skills
• Ability to communicate complex & distressing information to patients & their carers in a sensitive & supportive way at a level they can understand
• Ability to manage complex caseload of high risk patients
• Able to utilise research and audit to maintain and promote best practice

• Must be able to meet travel requirements of the post
