A care management company in Scotland seeks a Case Manager to assess rehabilitation needs and coordinate care services for clients with brain injuries. The ideal candidate will have a professional qualification in health or social care, strong communication and problem-solving skills, and at least 3 years of experience in their field. This full-time position is based in Edinburgh, with a salary of £40,000 to £45,000 depending on experience.

Salary: £40,000£45,000 depending on experience (To be discussed at interview)
As Case Manager, we work closely with clients and their families to assess rehabilitation needs, commission appropriate services, and lead multi-disciplinary teams, delivering tailored support within the community to help individuals achieve their optimum potential.
Brain injury case management is a highly specialised role that goes beyond coordinating care and involves comprehensive assessment of physical, cognitive, emotional, and social needs, setting meaningful goals, and ensuring interventions are evidence-based and client-focused. We advocate for our clients, navigate complex health, social care, and legal systems, and continuously monitor progress to adapt plans as needs evolve.
