A community health provider is looking for a Case Coordinator in Helston. The role involves coordinating patient care, managing community referrals, and ensuring efficient discharge processes. Candidates should have excellent communication skills and a Level 4 qualification in relevant fields. This is a full-time position offering a salary between £27,485 and £30,162 per annum.
An exciting and rewarding opportunity has arisen for a Case Coordinator to join the team in the West Integrated Transfer of Care Hub (iToCH). Currently based in Helston Community Hospital. There would be an expectation for you to hot desk and shadow staff during your induction at Camborne Health Office and Penwith Health Centre. You will play an important role in supporting clinical staff with admission avoidance, arranging community support, case managing non‑urgent referrals and facilitating safe and timely discharge from the acute and community hospitals for patients.
This is an ideal opportunity for an enthusiastic and dynamic individual. You will need to have good interpersonal skills, IT skills and possess excellent communication and organisational skills.
We will offer a supportive working environment in which to continue to develop your skills and knowledge in patient flow through a structured in‑service training programme, appraisal, and supervision systems.
You will help provide a seamless service for the people we serve which will include cross boundary and collaborative working with the multi‑disciplinary team, along with colleagues in Adult Social Care and other outside agencies in the community setting.
