A community support organization in England is seeking a Carer Wellbeing Assessor to conduct statutory and preventative assessments for adults caring for relatives. This home-based role requires strong communication and interpersonal skills alongside knowledge of health and social care. Occasional travel across West Sussex for meetings and home visits is expected. The ideal candidate will play a vital role in ensuring carers access appropriate support, making a meaningful impact in their lives.
We are currently offering a rewarding opportunity within our Carer Assessment Service, who are responsible for carrying out Statutory and Preventative Carer Assessments across West Sussex, and specifically to visit carers living in the mid Sussex area.
As a Carer Wellbeing Assessor, you will use your knowledge and / or experience of assessments, your working knowledge and / or lived experience of health and social care, and your excellent communication and interpersonal skills to carry out Carer Assessments under the Care Act 2014. You will provide access to the most appropriate and proportionate support to meet the identified need(s).
This is a home-based role with occasional travel across West Sussex for meetings and home visits.
While work is countywide, a base in the Mid Sussex area is preferred, as much of the work and any occasional home visits will be concentrated in this part of the county.
