A healthcare organization in the UK seeks a Care Navigator to join its friendly, busy practice. Responsibilities include managing patient enquiries, assisting with appointment bookings, and ensuring a smooth reception experience. Candidates should have experience in a GP surgery and be proficient with IT tools. This full-time role emphasizes patient care and team support.
We are looking for an outgoing and enthusiastic Care Navigatorto join our Reception and Administration Teamon a full time basis. We are a friendly, busy practice with a great team who strive for excellence in patient care and staff wellbeing.
You main duties will be in the practice reception area, undertaking patient administration, greeting and booking in patients at the front desk and supporting patients and staff at the surgery.
Document and Records Management
Managementof repeat prescriptions
Maintain clear working areas
Any other administrative tasks as required from time to time by the practice.
This is a busyinner-city practice of 9,900 patients. The practice has 3 Partners, 9 salaried GPs, a Pharmacy team, Nursing team as well as a dynamic Administration team. TheElms is a training practice for GP registrars.
The wider Patient Services Team at the practice is integral in ensuring quality care and experiences for patients when they visit and interact with the practice. Based within reception, this role is a vital first point of contact for patients and requires a friendly and personable disposition. You will be responsible for:
Thereception of patients at the front desk, and management of their movements inthe building.
Assisting with calling 999Ambulances
Administratingthe waiting room and appropriate systems.
Makingappointments for in house clinics and attached services.
Chaperones when required.
Registration of New patients.
Scanning and Read Coding of incomingmail on to patient electronic notes.
Summarising of Medical Records asper practice protocol where required.
Extract patients records and prepares other documents required for consultations.
Retrievemedical records if required by GP.
Orderingof Urine, Faeces samples, and Lab Forms.
Suppprt Medicines Management including reviews and prescriptions.
Liaisewith GPs and Reception/Administrative Team
Informingpatients of their test results.
AnyOther administrative tasks as required from time to time by the practice.
