A high-performance GP surgery in Bletchley is looking for a Care Navigator to join their reception team. The role requires assisting and directing patients, ensuring efficient service access, and supporting the practice team with various administrative tasks. Candidates should have previous experience in care navigation, be flexible to cover shifts, and possess knowledge of System One. This position offers a supportive work environment focused on staff development and patient care.
We are looking for a Care Navigator to join our reception team with a working pattern of 20 hours over four days Monday to Thursday AM shift (8am to 1pm) or PM shift (1:30pm to 6:30pm) with flexibility to cover shifts, including the opposite shift as needed for sickness or annual leave. The primary focus of the role is to receive, assist, and direct patients (care navigation) in accessing the appropriate service or healthcare professional in a courteous, efficient, and effective way.
Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
Receive and make telephone calls as required and take messages, ensuring accuracy of detail and prompt appropriate delivery.
Process and complete medical and admin triage forms.
Action repeat prescription requests.
Ensure that all new patients are registered onto the computer system promptly and accurately, including Temporary Residents.
Westfield Road Surgery is a 5 Doctor Practice caring for 8750 patients. We are located in Bletchley Milton Keynes. We have 4 Practice Nurses and 2 Health Care Assistants. We have an experienced management team to lead us.
We are one of the 4 GP practices which form the Southwest Primary Care Network.

To have a thorough knowledge of all practice procedures.

Receiving patients consulting with members of practice team
Appointments
Process medical and admin triage forms which are requested on a daily basis.
Computer
Registrations of new patients computer data entry and medical records.
