Job summary

King's College Hospital NHS Foundation Trust is seeking a Care and Support Navigator Service Lead at Denmark Hill to oversee discharge support for identified cohorts and to drive timely, safe patient transitions.

You will develop procedures, triage requests, train volunteers, and coordinate a patient-centred workflow while maintaining KPIs and collaborating with discharge coordinators and multidisciplinary teams.

Qualifications

• Master's degree or equivalent in a health-related subject with significant experience.
• Continuing professional development evidence.
• Conflict resolution training would be advantageous.

Responsibilities

• Establish and oversee discharge support service for identified patients.
• Develop SOPs and communication materials for elective pathways.
• Triage incoming requests and review with discharge coordinators.
• Engage with patients before discharge to assess support needs.
• Manage personal caseload and deliver outcomes per SOPs.
• Deploy resources to support timely discharge (keys, shopping, alarms, etc).
• Provide up to 4 weeks of ongoing support post-discharge (phone call or home visit).
• Train, support and deploy hospital and community volunteers.
