Overview

We have an exciting opportunity to join our team for those with a keen interest in the Brain Injury and Mental Health sectors.

The role would provide the right candidate the opportunity to become a Field Care Manager within our team. You will be managing packages of care for those with Mental Health needs and acquired brain injuries.

Responsibilities
  • Risk assessment and Care Planning around individual client needs.
  • Supervision and appraisal of staff
  • Monitor support packages including reviewing support worker documentation.
  • Communicating effectively with our clients, their families, staff teams and our colleagues in the multi-disciplinary team.
  • Rota management
