Job summary

PartnersHealth is seeking a Care Homes Clinical Lead (Advanced Practitioner) in Rushcliffe to clinically lead the PCN Care Homes Team across GP practices and care homes. You will combine clinical leadership, operational management, and hands-on patient care to improve outcomes for older residents.

You will supervise a multidisciplinary team, drive EHCH delivery, ensure governance and data-driven service improvements, and work across health and social care partners to deliver proactive, safe care.

Qualifications

• Senior clinical leadership or line management experience in health or social care.
• Experience working autonomously with older populations, frailty, or complex multi-morbidities.
• Experience conducting staff appraisals, clinical notes and medication audits.
• Experience managing relationships across multiple GP practices and care home operators.

About the role

Post Care Homes Clinical/Operational Lead

Reporting to GP Lead for Rushcliffe PCN Care Homes Team

Location Based at - Keyworth Health Centre (spanning 11 GP practices, and older persons care homes in Rushcliffe)

PURPOSE OF THE ROLE The postholder will operationally manage the Rushcliffe PCN Care Home Team, a multidisciplinary team (MDT) comprising GPs, advanced clinicians, nurses, and care coordinators. Working closely with Rushcliffe GP practices and care home staff, the postholder will deliver proactive care and support to older people living in residential care. This role bridges health and social care boundaries to deliver the NHS Enhanced Health in Care Homes (EHCH) framework.

Key Responsibilities

Operational Management & Leadership

• Service Delivery: Manage the team's delivery of the NHS Network Contract DES Enhanced Health in Care Homes specification.
• Staff Management: Conduct staff 1-2-1s, annual appraisals, clinical notes audits, and medication audits.
• Accountability: Support the team, hold staff accountable for action delivery, and directly address poor practice when necessary.
• Financial Efficiency: Ensure the care home team operates efficiently and provides value for money as a centralized PCN service on behalf of the 11 GP practices.
• Strategic Input: Contribute as an active member of the PartnersHealth senior leadership team, balancing operational duties with strategic organisational positioning.

Clinical Excellence & Practice

• Clinical Support: Provide hands‑on clinical care and autonomous decision‑making for care home residents following comprehensive assessments.
• MDT Collaboration: Contribute to weekly care home rounds, support the team and contribute to clinical decisions when needed.
• Safe Prescribing: As an independent prescriber prescribe safely and appropriately in accordance with local non‑medical prescribing policies and protocols.
• Governance & Risk: Apply strong clinical governance and risk management principles to all areas of practice, highlighting risks and benefits when developing new ways of working.

Data, Reporting & Service Development

• Data Utilization: Interpret data and local intelligence to drive service improvements, support primary care resilience, and reduce avoidable urgent and emergency service activity.
• Performance Reporting: Provide data‑driven reports to commissioners, the PCN and to the PartnersHealth Board to evidence compliance and delivery with local and national specifications.
• Innovation & Tech: Promote the adoption of digital technology where possible to enhance patient safety, increase operational efficiency, and establish best practices.
• Evidence‑Based Care: Assist in developing an evidence base for health needs‑led service planning, commissioning, and identified research developments.

Communication & Professional Standards
