Job summary

West London NHS Trust seeks a GMC registered GP to provide care in dedicated care homes within the Ealing borough. The role focuses on face-to-face weekly consultations in care homes, plus telephone triage and planned care, with robust support from a multidisciplinary team of nurses, prescribing pharmacists and administrators.

Qualifications

• GMC registration as a General Practitioner (GP).
• Commitment to patient-centred care.
• Interest in frailty or elderly medicine (can be developed in post).

Responsibilities

• Face to face routine weekly consultations in care homes.
• Telephone consultations for acute triage and planned care.
• Check and sign repeat prescriptions electronically.
• Prescribe generically in line with formulary, NICE guidance and Good Prescribing Practice.
• Handle queries, paperwork and clinical administration as required.
• Develop care and treatment plans with patients, care homes and relatives.
• Participate in clinical audits and practice meetings.
• Support other primary health care team members.
• Lead the practice in achieving performance indicator targets (QOF, DES, Enhanced Services).
