Are youlooking to challenge yourself and provide support and co-ordination to a small,but busy team of dedicated clinicians who provide care for our elderlypatients resident in care and nursing homes?

Anexciting opportunity has come up for a Care Co-ordinator to work as part of theBeacon Medical Group Care Home Team. This is a fixed-term appointment toprovide maternity cover for our current Care Co-ordinator.

Thesuccessful candidate will be self-motivated and able to work with minimumsupervision, planning and co-ordinating the day-to-day work of our Care HomeTeam as well as supporting Multi-Disciplinary Team Meetings, rota management,liaison with outside agencies, including care homes, District Nurse Teams, runningand compiling reports and other administrative tasks.

Main duties of the job

This isa varied role requiring good administrative, communication, numeracy andliteracy skills. This is for 28.5 hours over 5 days.

The mainduties of the role are:

Facilitate effective communication between care homes, patients, members of theprimary health care team, secondary care and other associated healthcareagencies.

Provide coordination and navigation through relevant pathways, liaising withlocal support services, professionals, and wider stakeholders.

Organisation and administration of MDT meetings, rota management, meetingplanning, minute taking, review of audits and training support.

Support to minimise the demands upon the multidisciplinary team.

Planning and co-ordination of the Care Home Service home/ward rounds, andclinics and tasks. Play an active rolein capacity planning and support.

Co-ordination and support of any reports and audits for the service

Tosupport and work closely with the management and Care Home Team providing acomprehensive administrative service.

Ensure administrative and clerical duties within the practice are carried outwithin the guidelines and policies for the Care Home Service.

Beresponsible for improving healthcare outcomes by helping with enhancedservices, QOF and IIF work in the practice.

The successful candidate will have a good level of computer literacy with workingknowledge of Microsoft Word and Excel. Ideally, they will be familiar withSystmOne or equivalent.

Person Specification
Qualifications
  • Evidence of a good general standard of education.
  • NVQ Administration level 3 or equivalent level of experience.
  • RSA level 1.
Experience
  • Experience of working with the general public.
  • Experience of working in a busy environment.
  • Understanding of different patients and carer groups and needs.
  • Awareness of the importance of safeguarding and procedures.
  • Be aware of confidentiality and data protection when handling sensitive information.
  • Developing productive and effective relationships with all members of the healthcare team.
  • High degree of organisation and an effective time manager.
  • Completing audits.
  • Data analysis and reporting.
  • Experience of working in within a General Practice environment.
  • Patient/customer service related training/ previous role.
  • Up-to date knowledge of services and health resources in the local area.
  • Knowledge of the GDPR and how this is applied in practice.
  • Good working knowledge and experience of using SystemOne.
