A major healthcare provider in the UK is seeking a Care Coordinator to deliver high-quality, patient-centered care within a multidisciplinary team. The role involves both clinical duties as a Health Care Assistant and administrative tasks for smooth operations. The Care Coordinator will play an important role within thePrimary Care Network (PCN), working as part of the multidisciplinary team todeliver high-quality, patient-centred care.
The primary focus of this role is to provide clinical support as aHealth Care Assistant (HCA) undertaking tasks such as blood pressuremonitoring, blood tests, ECGs, and other routine procedures.
In addition, as part of their role, the post holder will providesecondary support to the Care Coordination team, assisting with administrativetasks, reception cover, and rota coordination to ensure smooth day-to-dayoperations across the PCN.
This hybrid role requiresexcellent clinical competence, attention to detail, and strong communicationand customer care skills.

Primary Care Networks (PCNs) are a key part of the NHS Long TermPlan, with General Practices grouping together to form a network ofPractices, supported by a number of additional clinical roles.
The St Helens South Primary Care Network Limited Company (PCN) isthe organisation that delivers the core components of the PCN Contract as wellas supports the wider Primary Care system and tenders for, designs and developsnew/ existing services for the benefit of the St Helens South population.
The PCN is comprised of 10 Practices in the St Helens South areas.The practices have a combined geographic area made up of around 85,000patients.
This role will contribute to the improvingquality of care of our patients across the PCN and its network of practices.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
