Job summary

A Primary Care Network in Lancashire is seeking a Care Coordinator to support patients in preparing for clinical conversations and managing their care. You will work closely with GPs to identify needs and provide personalized care plans. Ideal candidates have experience in General Practice and possess strong organizational skills. This role requires flexibility, including possible evening and weekend hours.

Qualifications

• Experience of working in General Practice/Primary Care/Social Care.
• Experience of managing challenging conversations.
• Experience of working with vulnerable patients with complex needs.

Responsibilities

• Proactively identify and work with patients to support their personalised care requirements.
• Coordinate all of a person’s identified care and support needs into a single care plan.
• Work with End of Life patients to complete care plans.

Skills

Ability to work as part of a diverse team

Experience of managing conflicting workloads

Experience of working with EMIS systems

Ability to drive / have own car

Education

GCSE grade A to C in English and Maths

NVQ level 3 in a relevant topic
