West Kent Primary Care is looking to recruit a Care Coordinator to support Mote & Wallis Primary Care Network.
This role will support The Mote Medical Practice and Wallis Avenue Surgery in expanding capacity for patients to access a clinical professional. The practices have a combined patient population of around 17,000. The post holder will work across three sites alongside other PCN members of staff. The PCN also provides MSK and Children's Social Prescribing services.
The post holder will mainly be based at Staplehurst Health Centre, with some requirement to travel to Mote Medical Practice and Wallis Avenue Surgery.
Mote and Wallis PCN would like to recruit a proactive and enthusiastic Care Coordinator to support their practices in expanding services available to their patients. This role will require the post holder to support practices with achieving Quality Outcomes Framework (QOF), Investment and Impact Fund (IIF) Direct Enhanced Service (DES) requirements; full support and training will be given. In addition, you will be working closely with the clinical team to organise and monitor patient access to services in particular chronic disease clinics.

A focus on improving patient access to care
Support on digital front door offering for patients
Monitoring urgent 2 week wait referrals
Review of vaccination achievements and to assist in appointments for target groups
Lead on weight management referrals
Assist with referrals to multidisciplinary meetings
Assist with administrative workaround personalised care
Lead with Patient Participation Groups
The remit of this role is widening and may adapt as our practice priorities change.
