Job Scope

Our client is looking for a dedicated Care Coordinator to join their growing healthcare team. The Care Coordinator will play a key role in ensuring clients receive exceptional, person-centred care within agreed timescales. This role involves scheduling staff, maintaining accurate records, and supporting effective communication between clients, care professionals, and families.

The Care Coordinator will manage care rotas, ensure all visits are appropriately covered, and maintain compliance with CQC standards. They will work closely with field staff, providing them with up-to-date information and support to deliver quality care. The ideal candidate will be highly organised, confident using scheduling systems, and motivated to build strong relationships with both clients and staff. This role requires a compassionate approach, a clear understanding of healthcare legislation, and the ability to prioritise effectively in a fast-paced environment.

Do you have?
  • Previous experience working as a Care Coordinator or in a similar role within health and social care
  • Excellent organisational and communication skills
  • Experience using care management or scheduling systems (e.g., People Planner or similar)
  • Good understanding of CQC regulations, the Care Standards Act, and the Health and Social Care Act
  • Ability to manage priorities and handle pressure in a busy environment
  • A caring and professional attitude with strong attention to detail
  • Competence in using Microsoft Office and maintaining accurate digital and manual records
  • Flexibility to participate in the on-call rota when required
