CCH Group in the United Kingdom is seeking a Care Coordinator to organise and deliver high-quality community care from our Ashton-under-Lyne base. You will manage referrals, create rotas, liaise with social workers and GPs, and monitor service users' conditions on a daily basis.

The role offers Monday–Friday 9am–5pm with additional on-call duties, a salary of £25,364 plus on-call payments, and opportunities for career growth within a trusted home-care provider.

Qualifications

• Efficient and organized; excels at managing rotas to high standards.
• Experience in healthcare and understanding of CQC and care plans.
• Strong communication with care workers, social workers, and GPs.

Responsibilities

• Coordinate referrals and create rotas; liaise with professionals (social workers, GPs) and monitor service users' conditions.
