Overview

We are looking to recruit a Mental Health Practitioner /Care Coordinator to work in the Tameside and Glossop Early Intervention Team.

Team and setting

The Early Intervention team are a multi disciplinary team with staff from a range of professions including mental health nurses, CBT therapists and support workers. The team has a dedicated Consultant Psychiatrist.

Responsibilities
  • You would be responsible for a defined caseload of individuals experiencing first episode psychosis as a care coordinator / mental health keyworker within the team.
  • Work with individuals to meet their identified needs using outcome measures and a recovery approach.
  • Facilitate discharge from secondary mental health services, or referral on as appropriate.
  • Contribute to duty and clinic work and participate in team development issues.
