About the Role

We are seeking an experienced Care Coordinator to join a well-established domiciliary care provider. The successful candidate will play a key role in ensuring smooth day-to-day operations, delivering high-quality care, and maintaining excellent communication between clients, carers, and management.

Key Responsibilities
  • Schedule and coordinate care visits efficiently to meet client needs
  • Support care staff and maintain regular contact with clients and families
  • Ensure compliance with CQC standards and company policies
  • Manage and update care plans and documentation accurately
  • Assist with recruitment, induction, and supervision of care staff
  • Liaise with external professionals and agencies to support client wellbeing
  • Participate in on-call duties as required
Requirements
  • Minimum 2 years’ experience as a Care Coordinator within a domiciliary care setting
  • Strong organisational and communication skills
  • Knowledge of CQC regulations and compliance requirements
  • Proficient in care management systems and scheduling software
  • Full UK driving licence and access to a vehicle
