Care Coordinator - Childhood Immunisations and Vaccinations

We are willing to consider full-time and part-time applications.

HolgatePrimary Care Network is looking to recruit a Care Coordinator to join our existing team supporting our 3 PCN Practices with a population of 45k patients. The purpose of this role is to provide administrative & patient support coordinating and organising Childhood immunisations and Vaccinations (Such As but not limited to, Shingles & Pneumococcal). We are looking for an individual who is enthusiastic, highly motivated & who will contribute to our provision of high quality, patient centred care.

Main duties of the job

Applicants are expected to have exceptional organisation and patient care skills, and empathy regarding patient needs. You will coordinate the care needs of individuals as requested by the PCN Management Team. The post holder should have the ability to manage multiple tasks such as, booking appointments, managing IT and remote booking systems, telephone skills, coordinating clinics and record keeping. Due to the nature of this role, it is desirable (but not essential) that applicants have previous experience of working in a health care or medical environment. You will be required to undertake training as set out by the Personalised Care Institute.

About us

Holgate Primary Care Network (PCN) consists of the following 3 practices in the Middlesbrough area. Linthorpe Surgery, Borough Road & Nunthorpe Medical Group & Village Medical Centre. We have a highly skilled & varied PCN team consisting of many roles such as Mental Health Practitioners, Physiotherapists, Care Home Practitioners, Social Prescribers, Safeguarding Lead etc alongside a supportive management and Care Coordinator team.

Job responsibilities

Duties and responsibilities will include:

Using GP systems to identify the cohorts of patients who requires specific vaccinations and appointments.

Develop a thorough understanding of the vaccination programmes and targets that all practices within the PCN are required to meet.

Organising the booking of these identified patients using telephone, S1 practice and remote booking systems.

Coordinating immunisation and vaccination clinics across the PCN and 3 Practices.

To regularly monitor the appointment systems ensuring clinics are fully utilised and (as directed by management) take action to book all available appointments.

Support admin teams in practice by working with them to arrange immunisations / vaccinations and meet patient needs.

Escalate concerns to our safeguarding lead as required.

Review and always adhere to practice protocols and policies.

Attend training events organised by the PCN, where appropriate.

Any other duties as requested by the Care Coordinator line manager and PCN Management Team.
