Salary : £26,000 £30,000 per annum (DOE)

We are seeking an experienced Care Coordinator to join a rapidly growing healthcare provider in the West Midlands. This role offers a mix of office-based responsibilities and field visits, ensuring that clients with complex care needs receive safe, consistent, and high-quality support.

As a Care Coordinator, youll oversee care packages, manage staff rotas, and maintain compliance standards while building strong relationships with clients, families, and healthcare professionals. Key Responsibilities :
  • Manage and maintain staff rotas to ensure full coverage of care packages.
  • Support care plan reviews, risk assessments, and staff supervisions.
  • Conduct spot checks, audits, and competency reviews in the field.
  • Liaise with clients, families, and healthcare professionals to ensure person-centred care delivery.
  • Handle queries and complaints professionally and efficiently.
  • Maintain compliance with CQC standards across all operational areas.

Participate in a shared on-call rota.
