Kingsley Healthcare Group is seeking a Care Coordinator for Oaklands Care Home in Scole. As a Care Coordinator, you will play a critical role in ensuring that residents receive high-quality, person-centred care. Working closely with the care team to coordinate and manage care for residents, considering their individual needs, preferences, and goals. You will be responsible for organising your shifts, administering and recording medication, liaising with health professionals and recording to the residents’ computerised care plans. You will assist with the induction, training and supervision of support staff.
