We are looking for a compassionate and organised MH & LD Care Coordinator to ensure the smooth delivery of care across our services.

Job Purpose

MH & LD Coordinator is responsible for managing and coordinating care for individuals with mental health needs in our supported living services. You will work closely with service users, care staff, families, and mental health professionals to ensure tailored care plans are delivered, helping individuals achieve their personal goals while maintaining their mental well‑being.

Key Responsibilities
Care Planning & Coordination
  • Develop, implement, and review personalised care and support plans, ensuring they address the mental health needs and goals of each service user.
  • Coordinate the scheduling of care workers, ensuring the right staff are matched to service users based on their mental health support needs and preferences.
  • Collaborate with mental health professionals, such as psychiatrists and therapists, to ensure comprehensive care and support for service users.
Team Support & Development
  • Provide day‑to‑day support to the care staff, ensuring they are well‑equipped to manage the mental health needs of service users.
  • Lead team meetings, supervisions, and performance appraisals, addressing any training or development needs specific to mental health care.
  • Ensure the care team follows best practices in mental health support, promoting a person‑centred approach and maintaining a compassionate environment.
Compliance & Quality Assurance
  • Ensure all care services comply with CQC regulations, safeguarding, and mental health best practices.
  • Conduct regular audits and quality assurance checks to monitor the effectiveness of mental health support provided, addressing any areas of improvement.
  • Maintain accurate records, including care plans, progress reports, and communications, in compliance with legal and organisational standards.
Crisis Management & Problem Solving
  • Act as a first point of contact in managing mental health crises, ensuring timely intervention and support for service users.
  • Work closely with healthcare professionals, families, and crisis teams to manage complex situations and prevent hospital admissions where possible.
  • Handle concerns or complaints from service users and families, ensuring issues are resolved sensitively and effectively.
Stakeholder Engagement
  • Build and maintain strong relationships with service users, their families, and external mental health professionals to ensure a collaborative approach to care.
  • Advocate for the needs of service users in multi‑disciplinary meetings, ensuring their voices are heard and their mental health goals are prioritised.
  • Provide regular updates to families and professionals, ensuring transparency and clear communication regarding service users' progress.
Qualifications & Experience
  • Proven experience in a care coordination role, preferably within mental health or supported living.
  • Relevant qualifications in health and social care, such as NVQ Level 3 or higher (or working towards).
  • Strong understanding of mental health conditions, treatment plans, and recovery‑oriented care.
  • Knowledge of CQC regulations and compliance related to mental health care.
