Job summary

Overview

Care Coordinator

📍 Location: Rochdale
⏰ Hours: Monday to Friday, 9:00am - 5:00pm (on-call on a rota basis)
💰 Pay: £13.74 per hour
📄 Contract: Temporary ongoing (6 months minimum)

We're looking for an experienced, organised, and caring Care Coordinator to join our client in Rochdale. This is an exciting opportunity for someone who is passionate about putting people first and ensuring high-quality, person-centred care is delivered every day.

As a Care Coordinator, you\'ll be the link between clients, their families, and the team of dedicated healthcare workers — ensuring smooth day-to-day operations and the best possible outcomes for those we support.

Key Responsibilities
  • Coordinating the delivery of care using our digital care scheduling system
  • Adapting to changes in schedules, including staff availability and onboarding new clients into existing care runs
  • Ensuring healthcare workers attend staff meetings, supervisions, and training within CQC-required timescales
  • Assisting the Registered Manager in delivering a high-quality, compliant care service
  • Managing on-call duties on a rotating basis to support staff and clients out of hours
