DerbyshireDales Primary Care Network Ltd (DDPCN Ltd) is seeking a, proactive andcompassionate Care Coordinator to join our team. This role willsupport the delivery of person-centred care by working alongside our existingCare Coordinators to identify and address the non-clinical needs of patientswith complex health and social circumstances.The post-holder will provide operational and administrative support to the CareCoordination function, including the coordination of our Home Visiting Team andthe Home from Hospital / Transfer & Settle Service, ensuring smooth,timely, and joined-up care for patients across the PCN.

You will also provide cover during periods ofabsence within the Care Coordinator team, ensuring consistency of servicedelivery across practices.

Please note this position will require Saturday and Sunday working between 0800 and 1800 hours each week as part of the 5 day rota . Job share can be considered to maximise opportunities to meet this weekend working provision .

There may be opportunity to work remotely ( from home ) on set days within the 5 day rota .This can be discussed further at interview or upon appointment of the prefered applicant .

Main duties of the job

Administrative and Team Support

Provide general administrative support to the Care Coordination team including appointment scheduling, call handling, correspondence, and documentation.

Attend and contribute to MDT (multi-disciplinary team) meetings, taking notes and following up on assigned actions.

Provide cover and continuity during periods of Care Coordinator leave or absence.

Contribute to internal audits, evaluations, and service reviews to support continuous improvement.

General Care Coordination Support

Assist Care Coordinators in identifying and supporting patients who would benefit from personalised care planning, including those with long-term conditions, frailty, or frequent hospital admissions.

Support patients in understanding and navigating local health and social care services, promoting self-management and independence.

Maintain up-to-date and accurate records in clinical systems, ensuring compliance with GDPR and local governance.

Home Visiting Team and Transfer from Hospital Patient Coordination will be principal functions.

Manage daily referrals and scheduling for the PCN Home Visiting Team, ensuring efficient allocation of visits.

Liaise with GP practices, community services, and visiting clinicians to prioritise and coordinate patient visits.

Track visit outcomes and ensure follow-up actions are communicated and documented.

Weekday and Weekend working will form part of the holistic Care Coordinator rota .( Day shifts working agreed hours between 0800 and 6.30 p.m.)

DDPCN supports the 11 GP Practices situated across the beautiful Derbyshire Dales .

The GP Practices are geographically located from Asbourne in the South Dales up to and including Castleton and Hope in the north Dales .

The main office is at Scholes Mill , Tansley near Matlock which has plenty of free on site parking .

The successful applicant will be joining a friendly and supportive team that has excellent working relationships with all our stakeholders and other community healthcare providers that we work in partnership with every day .

Job responsibilities

Thisis a full time role with flexibility around working hours.- Some travel across the Derbyshire Dales area may be required.- The role may occasionally involve attending meetings at partner sites.- Full training and supervision will be provided.

Weekend working (day shifts) may be involved in a rota basis . Therefore applicants need to be willing to work at a weekend when required to do so .
