Job summary

Allscreens Nationwide Ltd is seeking a practice co-ordinator to support patient care coordination within a primary care setting. You will handle administrative duties, process medical letters, triage requests, and liaise with hospitals and community providers to coordinate care.

You will signpost patients to services, support vaccination and screening programmes, and ensure accurate medication records and regular audits of the clinical system.

Responsibilities

  • Administration duties - scanning and filing incoming post and electronic clinical letters.
  • Handle incoming practice email queries from hospitals or care agencies.
  • Manage admin requests through ANIMA (Triage system) for medical letters and report requests.
  • Assist with signposting patients to community/local services with social prescribers.
  • Act on communications from hospitals or community providers.
  • Follow death protocol procedures for families and relatives.
  • Review new cancer diagnoses with in-house social prescriber for cancer care review.
  • Manage incoming medical reports and SAR requests from patients and third parties.
  • Assist with patient participation in local or national screening programmes.
  • Promote health education, e.g. Self-Care Week participation.
  • Encourage vaccination opportunities (flu, shingles, childhood immunisations).
  • Organise and prioritise own workload.
  • Monitor tasks via regular audit of the clinical system.
  • Liaise with multi-agencies to coordinate patient care.
  • Support patients and carers and collect experience feedback.
  • Support PCN practices to deliver on QOF, KPIs and enhanced services.
  • Update and maintain accurate patient medication records on the practice system.

Job description

PURPOSE

To provide a practice point of contact for patients in coordinating their care To ensure timely follow up and action for patients from communications from community or secondary care To deal with incoming queries from patients and other healthcare providers To organise in-practice appointments, follow ups or other actions to help the PCN practices provide high quality, compassionate care to our patient population.

SPECIFIC JOB RESPONSIBILITIES

  • Assist with signposting patients to appropriate community or local services, alongside social prescribers.
  • Act on communications from hospitals or community providers.
  • Action and follow death protocol procedure allowing a smooth process for families or close relatives.
  • Review any new cancer diagnosis patients and consult with our in-house social prescriber for cancer care review.
  • Manage incoming medical reports and SAR requests (medical record requests) from patients and third parties.
  • Assist with and ensure patients participate in local or national screening programmes.
  • Promote health education to our patient population, such as through participation in self-care week.
  • Pro-actively ensure patients participate in vaccination opportunities such as flu, shingles, or childhood immunisations.
  • Organise and prioritise own workload.
  • Monitor tasks to ensure they are completed and care delivered through regular audit of the clinical system.
  • Directly liaise with multi-agencies to coordinate care for patients.
  • Support patients and carers and collect feedback on their experiences.
  • Support the PCN practices to deliver on QOF, KPIs and locally commissioned enhanced services.
  • To update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken.
  • Participate in practice meetings, patient participation groups and other meetings to improve engagement with the role.
