A healthcare provider in Ipswich is looking for a Care Coordinator to work within their multi-disciplinary team. This role involves supporting individuals and families by coordinating care across services, processing referrals, and improving patient support. Ideal candidates should have healthcare experience and be educated to GCSE level. Flexible working is available with a focus on reducing hospital admissions.

We are now seeking a Care Coordinator to work as part of this multi-disciplinary team. Youll support the co-ordination of support to individuals and families across multi-disciplinary services within the Care Management Service. This involves processing referrals and ensuring patients get the right support from the multidisciplinary service at the right time. Youll be the central point of information and guide processes to improve working practices.
