A healthcare provider in the UK is seeking a Care Coordinator to manage care packages and support clients in their homes. This full-time role requires proven care coordination experience, excellent organizational skills, and a full UK driving license. You will serve as the main contact for clients, ensuring care delivery aligns with their needs. In this pivotal role, you will be responsible for the planning, coordination, and ongoing management of care packages, ensuring seamless service delivery to our valued clients.
You will be based at your local Apollo Home Healthcare office in Albrighton, Wolverhampton, but at times, travel across the West Midlands & Shropshire regions will be required.
We are seeking an enthusiastic and outgoing individual with proven care coordination experience. Whether in complex care, domiciliary care, or another setting, you must be able to demonstrate your ability in scheduling, rota coordination, and effective team working.
