Primary Care Network 5(PCN5) is looking for an enthusiastic,reliable care coordinator to join our team in supporting our patients andclinical staff at the surgery & PCN.

We are seeking someone who enjoys working part of a team aswell as independently, who can multi-task and thrives in a busy workingenvironment.

This is a varied role and a keen eye for detail isessential. Flexibility is important to be able to cover colleagues duringperiods of sickness or holidays.

Training will be given in all aspects of the job, so thesuccessful candidate must be keen to learn new skills and enjoy a varied role.Candidates must be confident using IT.

This is a full time role for 37.5 hours.

Main duties of the job

This role will involve a range of tasks including but not limited to:

  • navigatingpatients to the right service
  • assistingwith the patient recall processes
  • Booking patients into clinics
  • Informing team when appointments slots are unutlisied
  • assisting with general patient queries
  • managingmedical records
  • referrals
  • dealingwith incoming clinical correspondence ensuring it is actioned appropriately
  • chasingclinical correspondences when test results are not yet returned
  • Supportin running extended access services (EAS)
  • Openand close GP surgery.
About us

Primary Care Network 5 (PCN5) Is based in Barnet, with a grouping of 6 GP practices across, Hendon, Golders Green & Cricklewood, with a list size of 50,000 patients.

PCN5 Has a wide variety of clinical and non-clinical stuff such as, MSK practitioners, ACP, Dietician, Mental Health practitioner, social prescribing link workers, GP & Pharmacists.

Job responsibilities

Prescriptions

Ensuring when prescriptions are authorised by clinicals that patients are informed which pharmacy it has gone to

Signing in patients into their appointments

Informing patients of late procedure & trying to reschedule where possible

Printing request forms for patients

When clinical information come through emails that it is forwarded to correct correspondent

When test results have not been received to contact the correct department and request it

Working Pattern

Supporting Extended access service by participating in evening shift and 1 a month a weekend.

Identify patients requiring routine reviews and vaccinations and book appointments with appropriate clinicians

Medical Records

Ensuring patients details are correct recorded

Other

Responding to patient queries

Provide cover for members of the Admin team during periods of sickness and annual leave which may include working additional hours/days.

To develop skills and competencies in new software/processes as required

Any other duties commensurate with the role, as directed by the Line Manager or PCN Manager.
